Endoscopic Sinus Surgery Modified Endoscopic Lothrop Procedure
This rarely indicated procedure involves drilling through the floor of the frontal sinus on each side and resecting part of the superior nasal septum and inferior frontal intersinus septum. The Modified Endoscopic Lothrop Procedure (MELP) is useful for patients with recalcitrant frontal sinusitis which persists after appropriate medical therapy and functional endoscopic sinus surgery techniques have been exhausted. This procedure is also indicated in neoplasms of the frontal and ethmoid cavities. The surgical video here demonstrates the MELP in a patient with persistent chronic frontal sinusitis despite several months of aggressive medical therapy and two prior functional endoscopic sinus surgeries. Each frontal recess is first identified. The superior nasal septum is then resected with a microdebrider and through-cutting forceps. An angled drill in conjunction with 30 and 70 degree endoscopy is used to enter the frontal sinus through the floor. Both frontal sinuses and frontal recesses are connected by drilling away the intervening frontal intersinus septum and agger nasi bone. Great care is taken to avoid the adjacent cribriform plate and dura.
